Project Description
The proposed project is to repair Barkaboom Culvert located in the Town of Andes, Delaware County within the Pepacton Reservoir on BWS Road No. 8. Barkaboom Culvert (BIN none) is a single barrel, cast-in-place, reinforced concrete box culvert, which conveys the Barkaboom Stream, a class A(TS) stream to the reservoir. This culvert is one of eleven structures planned to be rehabilitated in the Delaware Watershed under a single construction contract, DEL-150. Construction work on this contract is expected to take three years, with 6 months planned for Barkaboom Culvert site, starting in 2013. The structure exhibits structural deficiencies such as concrete spalling, horizontal and vertical cracks, mapcracking, efflorescence, honeycombing, scaling, exposed rebars, exposed wingwall footings and eroded stream banks. The scope of work for this construction contract is to provide a minimum 15 year service life for the structure at completion of construction.

DCAS is seeking the removalof a deed restriction on Lot 58 which restricts the use of the subject property to a 'non­ profit organization' only. The intent of the removal of the use restriction is to facilitate a potential development of the property pursuant to zoning. For the purposes of completing this EAS Short Form, DCAS is evaluating the reasonable worst case scenario of an as-of-right residential development (Maximum Floor Area Ratio: 6.02) on this site. DCAS determined that given the zoning, surrounding land uses, and dimensions of the lot,the reasonable worst case development scenario would be a 5-story,3-unit residential walk-up building,with a footprint of 1,260 square feet. The building height would be less than 50 feet. While the FAR and height restrictions may permit a sixth floor, the lot size would not easily permit an elevator,which would make a building of more than five stories unlikely. The proposed restriction removal is a site-specific action.

Project Description
The New York City Department of Homeless Services (DHS) is proposing to enter into a long-term contract with the Acacia Network (Provider) to operate a transitional residence (Shelter or Facility) for homeless adult women, located at 40-03 29th Street in the Long Island City neighborhood of Queens. The Facility is located in an approximately 45,000 gross square-foot, six story building (Building or Project Site) that was formerly used as a hotel, on an approximately 8,300 square-foot tax lot. The Facility would contain 74 units for adults, with an expected total residential population of 200 homeless women who would be referred to the Facility by DHS. Pursuant to the long-term contract, homeless adults staying at the Shelter would be provided with comprehensive on-site social and other services to assist them in obtaining permanent housing and remaining stably housed in the community. There are expected to be a total of approximately 40 program and security staff members at the Facility over three daily shift periods. Seven off-street parking spaces will be provided to staff members as part of the Facility. Minor interior renovations are expected at the Project Site as a result of the proposed action.
