Pursuant to FOIL, please provide the following records related to the 11,049 deaths processed by the OCME on April 28-30, 2020 (1,998, 6,533, and 2,518 deaths, respectively). Note, these figures are from records provided to me by OCME on 8/23/23 in response to my 5/31/2023 request: 1) An Excel spreadsheet showing the month/day/year on which each these 11,049 deaths occurred. 2) The places of death for these 11,049 deaths, i.e., the number that occurred at hospitals, at nursing homes/long-term care facilities, at personal residences, and in other places. 3) The number of these 11,049 deaths that attributed covid-19 (ICD code U07.1) as underlying cause. 4) The number of these 11,049 deaths that attributed another non-covid respiratory disease as underlying cause (ICD codes J00-J99) 5) The number of these 11,049 deaths that attributed a circulatory disease as underlying cause (ICD codes I00-99) 6) An age group breakdown of these 11,049 deaths. Please do not exceed a 10-year bracket, with the exception of pediatric ages and age 85+.

Previously, OCME has released the following fields related to deaths and, given this attached release, we would request the same fields. We would also request a data dictionary of all fields maintained by OCME, separate from those being released, along with exemptions cited that prohibits the release of those fields: Date of death; time of death; gender; city of death; state of death; place of death; zip code; age; immediate cause of death; secondary and third causes of death; and other significant causes of death.
